Respiratory disease is a major cause of mortality and morbidity, and represents a high chronic disease burden nationally. Acute-care management of respiratory disease, particularly Chronic Obstructive Pulmonary Disease (COPD) is costly and inefficient. The Comprehensive Respiratory Care (CRC) Program is an innovative, pragmatic strategy designed to serve as a bridge between acute and community based care. This webinar is focused on how the CRC program provides evidenced-based care to respiratory patients utilizing the true fundamentals of health: Education, empowerment, and self-management. The program has multiple service offerings delivered by a multidisciplinary team all focused on preventing acute care utilization and helping patients living with respiratory disease have a better quality of life.
