Join us as we delve into the collaborative efforts of PathwaysBC and Newfoundland's Family Practice Networks to implement a single source of truth for referral information. Pathways is a trusted online directory of healthcare providers, services, and clinical resources that supports clinicians. Pathways supports delivery of timely, high quality patient care, and helps reduce clinicians' administrative burden contributing to a stronger health system. This presentation will explore the origin of PathwaysBC's organizational growth, the inception of partnership between the two provinces and a brief platform demonstration of the platform, followed by a Q&A session.

Deborah Wearn

Deborah joined the RE-Boot FPN as Executive Director in 2020. With 25+ years in the non-profit sector across NL and Ontario, she excels in stakeholder engagement and leadership.


Adam Wylie, Endeavor FPN Executive Director

Adam Wylie joined the Endeavor FPN as Executive Director in 2019. Previously, he worked in British Columbia, strengthening primary care and fostering collaboration among health care, non-profit, and government organizations.


Ryan Lammertsen, Executive Director of the Pathways Patient Referral Association

Ryan Lammertsen has led the Pathways team for the last 10 years, starting from a small pilot project in the Lower Mainland of British Columbia to what is now a widely used directory and resource for clinicians across BC. Ryan has a background in public health, receiving a MPH from the University of British Columbia. He also has served in a variety of roles in the Divisions of Family Practice.


Melissa Faraguna, PathwaysBC Provincial Administrative Support Manager

Melissa Faraguna is the Administrative Support Manager for PathwaysBC, where she has worked since 2014. She brings over 25 years of experience in Family Practice, supporting physicians both clinically and administratively, and has also worked with Divisions of Family Practice.
