The Canadian College of Health Leaders is proud to announce that the 2020 Recognition in Delivering Value-based Healthcare has been awarded to Women’s College Hospital and their Surgical Team – Total Joint Replacement Program.
The Total Joint Replacement Program is a unique collaborative aiming to deliver effective, efficient, and patient centered care for individuals with hip and knee arthritis. To shift a traditionally in-patient procedure to out-patient care, the team developed an innovative anesthesia protocol, as well as a virtual care program to monitor and support patient recovery at home.
This program allows patients to recover in the comfort of their own homes with their families. It also demonstrates value-based healthcare by helping to alleviate some of the burden on the healthcare system by leaving hospital beds open for patients in need of acute care. In the traditional in-patient setting, hip and knee replacements require patients to spend an average of three nights in the hospital at a significant cost to the health system. The team has developed new care pathways and virtual tools that are being shared with hospitals across the province. This program is changing the way that care is delivered, resulting in improved patient outcomes and value for the healthcare system.
The College’s Recognition in Delivering Value-based Healthcare aims to increase the profile and understanding of value-based healthcare by honouring an organization, or team, that is deliberate in changing the way that care is delivered, resulting in improved patient outcomes. This means that patients are being optimally cared for at the right time, in the right setting and at the right cost.
