Congratulations to the 2026 Solventum Health Care Innovation Team Awards Recipients

The Canadian College of Health Leaders and Solventum (previously 3M Health Care) are proud to announce that the recipients of this year’s Solventum Health Care Innovation Team Awards are Waypoint Centre for Mental Health Care and CHU Sainte-Justine.

Waypoint Centre for Mental Health Care

Waypoint Centre for Mental Health Care is the recipient of the Solventum Health Care Innovation Team Award (Disruptive innovation initiatives across a health system), recognizing the organisation for its bold reimagining of how mental health and addiction (MH&A) care can be delivered across a health system. Through Waypoint at Home, Ontario’s first MH&A Home Care program, Waypoint has fundamentally shifted care from hospital to community, breaking down long‑standing barriers that have left many patients without appropriate post‑discharge support. By introducing broad eligibility, collaborative care planning, and flexible, person‑centred services, Waypoint created a model that disrupts the “normal” pathways of care and delivers measurable, system‑wide impact.

In just its first year, the program achieved an 88% reduction in MH&A admissions and a 0% 30‑day MH&A ED revisit rate - compared to a regional average of 24% - while also reducing inpatient length of stay and generating more than $1 million in system savings. The model’s rapid spread to additional Ontario settings, growing Community of Practice, and ongoing evaluation using implementation science position it as a scalable, sustainable innovation that is reshaping MH&A care across the continuum. Waypoint’s leadership in this space reflects a deep commitment to system integration, value‑based care, and building healthier communities for the future.

CHU Sainte-Justine

The CHU Sainte-Justine’s Centre de recherche Azrieli research centre has been awarded the Solventum Health Care Innovation Team Award (Disruptive innovation initiatives within an organization) for its for its groundbreaking PRISME initiative. Confronting one of the most pressing public health challenges in Canada, the timely and equitable diagnosis of neurodevelopmental disorders, CHU Sainte‑Justine has reimagined the diagnostic pathway through a bold fusion of immersive technologies, explainable artificial intelligence, and seamless interoperability with electronic medical records. The result is a transformative model that reduces delays, enhances clinical precision, and improves the experience of families across Quebec’s culturally, geographically, and socio‑economically diverse communities. Developed through years of translational research and driven by CHU Sainte‑Justine’s commitment to humanizing care, PRISME offers a 15‑minute mixed‑reality assessment that engages children, reduces anxiety, and generates contextualized, clinically aligned insights. Its multimodal neural network, trained on a cohort reflective of Quebec’s population, produces differentiated outputs for clinicians and families, strengthening understanding, adherence, and continuity of care. By addressing long‑standing barriers such as fragmented services, decontextualized tools, and lengthy wait times, PRISME embodies the disruptive, scalable, and sustainable innovation envisioned by the Quebec health and social services network.

These awards, sponsored by Solventum, are part of the CCHL National Awards Program and will be presented during a luncheon at the CCHL National Conference on June 16, 2026.

About the Innovation Awards

Starting in 2024, the Solventum (previously 3M) awards’ focus shifted from quality improvement to disruptive innovation, to highlight cutting edge system transformation and outcomes, diverse population, and sustainability of change. These awards will continue to be presented in two categories, which have been updated to: Disruptive innovation initiative(s) within an organization, and Disruptive innovation initiative(s) across a health system. Changes and updates were made to the award criteria that include removing the minimum of 18 months of evidence of sustainability and focused instead on addressing long-term social and environmental needs to the organization/system.
