British Columbia is currently implementing a model of Primary and Community Care that enables integrated community-based care and services and provides access for the population “closer to home”. Specific to this transformation, the future vision includes the development of Primary Care Networks, that support patient access to team-based full service primary care clinics, Urgent and Primary Care Centres, and are formally linked to health authority and community based services/NGOs. Ted’s presentation focus’ on the Primary Care development and early implementation as this innovative model takes shape across the province.
