Healthcare Management Forum – Call for abstracts on Chronic Disease Management

Healthcare Management Forum is planning a special edition on Chronic Disease Management (from a leadership lens) for January 2027. Chronic diseases are a significant public health issue in Canada and health leaders play a pivotal role in making lasting systemic and patient-centred improvements.

Authors are invited to submit a 200-word abstract to lwilson@cchl-ccls.ca by April 1, 2025. Abstracts should include a short overview of the proposed manuscript, the intended audience, and key messages for readers. Full articles are due for peer review by May 1, 2026. The editorial staff will consider the following topics:

Strategic Planning in Chronic Disease Management

  • Designing and implementing sustainable care models for managing chronic diseases.
  • Integrated care systems/interdisciplinary collaboration that promote care coordination across different care sectors/providers to manage chronic conditions effectively.
  • Patient-centered care leadership that shifts toward prioritizing patients’ preferences, needs, values, and care goals.

Health Policy and Advocacy

  • Leadership advocacy and influencing policy to prioritize chronic disease prevention, early detection, and comprehensive management.
  • Leading community efforts to prevent onset of chronic diseases through lifestyle modification, vaccination, and early detection programs.

Innovation and Technology Integration

  • Leaders driving adoption of telemedicine and remote health monitoring for chronic disease patients, especially in rural/remote and underserved areas.
  • Leading integration of digital tools (e.g., wearable devices/apps) that empower patients to manage chronic conditions.

Health Equity and Access

  • Leadership in ensuring equitable access to chronic disease management services for underserved populations, including racial, socioeconomic, and geographic disparities.
  • Leading initiatives to address social factors that impact chronic disease management outcomes.

Chronic Disease Management Metrics and Evaluation

  • Measuring effectiveness in defining and tracking key performance indicators (KPIs) to assess the effectiveness of chronic disease management programs.

If your topic does not fall into one of these categories but deals with chronic disease management from a leadership lens, please feel free to submit an abstract. All potential authors will be contacted through e-mail by the Managing Editor.

Healthcare Management Forum (HMF) is the official peer-reviewed journal of the Canadian College of Health Leaders. The journal publishes articles on leading practices related to health leadership and management, including recent research, new technology and professional practices from health leaders’ perspectives. It is circulated to a variety of environments, such as medical companies, health authorities, multi-level care facilities, hospitals, public and private health agencies, health charities, the Canadian military and all levels of the Canadian government.
