Join this session to explore Newfoundland and Labrador’s ambitious journey toward integrated, team-based primary care. In partnership with Newfoundland and Labrador Health Services, the Family Practice Renewal Program, and the Health Innovation Group, this presentation will highlight how practice facilitation is driving greater integration within the Family Care Team model. Participants will gain valuable insights into the NL approach, exchange lessons learned from implementing team-based care on the ground, and generate ideas on how practice facilitation can support similar healthcare transformations across the country.

Paula Hanrahan, RN, BN, Masters, Nursing, Senior Program Consultant, Practice Improvement Program

Paula Hanrahan is a Senior Program Consultant, responsible for planning, developing, implementing and evaluating Practice Improvement Program activities. Paula takes a lead role in designing and developing evidence-based education and training and practice improvement supports across the three program streams.


Mark Watt RN, BN, MSc – Director of Innovations, Health Innovation Group

Mark Watt developed an interest in quality improvement and change management while working clinically as an emergency room nurse for 10 years. As an early adopter of quality improvement initiatives, an advocate for team based care, and by conducting research on emergency department care, he recognized the importance of supporting those doing the work to change the work. Stepping out of the clinical role, Mark has served in leadership positions focused on supporting physicians and teams to engage with quality improvement in pursuit of better patient and provider satisfaction, better patient outcomes and better value to health systems. Mark has consulted with organizations, provincial, national and international health systems to support their transformation. Mark has expertise in practice facilitation, measurement and project management skills including strategic partnership management/ stakeholder relations, program and initiative development, and evaluation of outcomes.

Justin Swain – Provincial Manager Process Improvement, Newfoundland and Labrador Health Services

Justin Swain is a Certified Lean Six Sigma Black Belt with over 25 years of experience leading transformational improvement across healthcare systems, public service, and industry. Currently serving as the Provincial Manager of Lean Process Improvement, Justin has been instrumental in embedding a culture of continuous improvement within healthcare by training more than 2,000 staff, including clinicians, executives, and frontline teams.

With deep expertise in Lean methodology, change leadership, and strategy deployment, Justin has facilitated dozens of Kaizen events and coached healthcare teams on redesigning clinical and operational processes to reduce waste, improve patient flow, and enhance care delivery. He developed and leads a comprehensive Lean Management System training program, customized for the healthcare environment, that is used across the province to build internal improvement capacity.

Justin is a respected speaker at quality improvement and patient safety conferences, known for making complex concepts accessible, engaging, and directly applicable to clinical practice. His published work on breast cancer diagnostic pathways underscores his commitment to patient-centered care and evidence-based change. He is especially passionate about supporting leadership development in the Family Care Team model and building system resilience through frontline empowerment and data-driven decision-making.
