Michael Garron Hospital (formally the Toronto East General Hospital), is a vibrant 450-bed community teaching hospital serving one of Canada’s most diverse communities in the heart of East Toronto. As a significant member of East Toronto Health Partners, the Ontario Health Team (OHT) serving East Toronto, MGH delivers health services to 400,000 people and has recently expanded its facilities with the opening of an 8-story Patient Care Tower to better service its growing community.  MGH is passionately committed to equity, diversity and inclusion.

Affiliated with the University of Toronto, MGH is seeking a visionary, forward thinking strategic leader who will serve as a role model in supporting the hospital’s strategic direction and, in addition to being an excellent clinician, be a visionary, operationally excellent and highly collaborative leader in the newly-created Department of Transitional Care.

Chief of Department of Transitional Care

With joint accountability to the Chief of Staff, VP Medical and VP Clinical, the new Chief will be the medical leader, in a productive dyadic leadership model for 160+ inpatient beds, including long-term care, palliative care, rehabilitation, memory care, and long-term ventilation. This leader will be operationally excellent and experienced in the management of inpatients, working with inter-professional teams, and in supporting patient flow in the hospital and with our community partners.  They will excel at addressing system capacity pressures, integration and seamless transitions for our patients.  Leading various inpatient divisions with diverse needs and reflecting their voice, the leader must be able to develop goals and objectives, prioritize projects, engage with and unify teams, and advise hospital leadership about strategic direction.  To excel in leading diverse and innovative programs, the Chief will be an energetic and charismatic leader, skilled at networking, who can engage with internal and external partners to create state-of-the-art programs that will grow in scope and size to better serve our community.  The leader will be familiar with senior-friendly strategies, supporting complex chronic conditions, and primary care system integration inside and outside the walls of our hospital.

MGH excels as an anchor partner in our Ontario Health Team, and our incoming Chief will have the ability to influence this system work.  The successful candidate will be an innovative and creative leader, able to develop and execute models of care that support the many life transitions of our patients and that focus on ensuring our patients are in the best environment for their needs, as those needs evolve.

MGH is one of the largest teaching sites for the University of Toronto Department of Family & Community Medicine and the incoming Chief will build on and grow our success as a highly sought-after teaching hospital with an opportunity to offer varied inpatient training opportunities, including for family medicine residents, physician assistants, and nursing trainees.  We are growing our research institute focused on patient-oriented care, and our incoming leader will have a passion for fostering a research and quality improvement mindset to our clinical work.

The new Chief will possess a CFPC, be eligible for licensure in the Province of Ontario and be eligible for an academic appointment at the University of Toronto.
