The Canadian College of Health Leaders is proud to announce that the 2024 Recognition in Delivering Value-based Healthcare has been awarded to the CIUSSS Centre Ouest de l’Île de Montreal for their value-based healthcare model for colorectal cancer.
Faced with escalating cases amidst resource constraints, the CIUSSS Centre Ouest de l’Île de Montreal ingeniously devised an Integrated Practice Unit (IPU), engaging patients in governance and process improvement. Through collaborative efforts with stakeholders like Coalition Priorité Cancer and the Rossy Cancer Network, a patient-centered approach was established, integrating clinical practitioners and patient representatives across the care continuum.
By implementing common indicators and streamlined processes, they ensured shared accountability and monitored outcomes effectively. Notably, their emphasis on patient-reported outcomes and experience led to tangible improvements in care delivery. This transformative model not only optimizes resource allocation but also sets a benchmark for value-driven healthcare nationally.
The College’s Recognition in Delivering Value-based Healthcare aims to increase the profile and understanding of value-based healthcare by honouring an organization, or team, that is deliberate in changing the way that care is delivered, resulting in improved patient outcomes. This means that patients are being optimally cared for at the right time, in the right setting and at the right cost.
This recognition is made possible by a grant from Medtronic Canada.
